WoofyFit Mobile Dog Gym Pet Service 

WAIVER AND RELEASE OF LIABILITY FORM

 

I, "The Client", participant in WoofyFit Mobile Dog Gym Pet Service, hereby acknowledges and agree to the following terms and conditions:

 

Assumption of Risk:

I understand and acknowledge that participating in physical activities with my pet, including but not limited to treadmill exercises, agility training, and other fitness activities provided by WoofyFit Mobile Dog Gym Pet Service, involves inherent risks. These risks may include, but are not limited to:

 

Injuries to my pet such as sprains, strains, and fractures.

Health issues or complications that may arise during or after physical activities.

Interactions with other animals that may result in injuries or altercations.

Unforeseen incidents, including environmental factors such as weather conditions.

I voluntarily assume all such risks and understand that WoofyFit Mobile Dog Gym Pet Service is not responsible for any injuries, illnesses, or damages that may occur during or as a result of my pet's participation in WoofyFit Mobile Dog Gym Pet Service.

 

Health and Vaccination:

I confirm that my pet is in good health, up-to-date on all vaccinations, and suitable for the physical activities provided by WoofyFit Mobile Dog Gym Pet Service. I am responsible for informing WoofyFit Mobile Dog Gym Pet Service of any health issues or concerns related to my pet.

 

Release of Liability:

I, on behalf of myself, my heirs, executors, administrators, and assigns, release, discharge, and hold harmless WoofyFit Mobile Dog Gym Pet Service, its owners, employees, and representatives from any and all claims, liabilities, demands, actions, or causes of action, whether at law or in equity, arising out of or related to any loss, damage, or injury that may occur during or as a result of my pet's participation in WoofyFit Mobile Dog Gym Pet Service.

 

Emergency Medical Treatment:

In the event of an emergency, I authorize WoofyFit Mobile Dog Gym Pet Service to obtain medical treatment for my pet. I understand that WoofyFit Mobile Dog Gym Pet Service will make reasonable efforts to contact me in case of an emergency.

Photography and Marketing:

I grant WoofyFit Mobile Dog Gym Pet Service the right to use photographs or videos of my pet for marketing and promotional purposes, including but not limited to social media, website, and other advertising materials.

 

I have carefully read and voluntarily signed this Waiver and Release of Liability Form, and I understand its contents.
